NCI Well Connect Mid-Week Brief
June 13, 2018
Dear Steve,

DID YOU KNOW that CYP1A2 gene expression affects athletes who use caffeinated beverages to boost athletic performance?

A study in the March issue of Medicine & Science in Sports & Exercise showed that fast caffeine metabolizers saw a significant boost in performance. Whereas partially slow and slow caffeine metabolizers saw worse performance, especially as the amount of caffeine intake rose.

This is significant considering caffeine is the most common stimulant used by athletes of all ages to improve performance.

Yet another reminder that we are not one-size-fits-all

Note: Our Pure Genomics panel screens for CYP1A2.

Diet Advice: Docs Willing But Unable
Bonnie and Steve: What do we hear at least once a week from new clients: "How come my doctor didn't know that?"

When it comes to getting medical nutrition advice, your doctor's ability to deliver is woefully inadequate. Much of the blame should not fall your doctor's shoulders, however.

As Dr. Stephen Devries, our favorite (non-practicing) preventive cardiologist likes to say, you cannot be held accountable for what you are not taught.

Evidence suggests that physicians are willing to discuss nutrition, yet many don't, often citing insufficient knowledge and training as barriers.

For example, in a recent survey, only 14% of internal medicine trainees said they were confident in their ability to counsel their patients about diet. In a survey of cardiology fellowship directors, only 44% said they provided a nutrition lecture, and only 27% of their chief fellows recalled a nutrition topic.

Dr. Devries is putting his money where his mouth is. He is making it his life's work to educate doctors about how to discuss diet and lifestyle with their patients. He has a very difficult job ahead of him. However, the American Heart Association (AHA) may have made his job a bit easier.

Last week, the AHA published advisory, "Medical Nutrition Education, Training and Competencies to Advance Guideline-Based Diet Counseling by Physicians," in the June 5th issue of Circulation

While we do not agree with many of the AHA recommendations, it is a start!

In addition, major reforms in undergraduate and graduate medical education designed to incorporate advances in the science of learning and to better prepare physicians for 21st century healthcare delivery are providing a new impetus and novel ways to expand medical nutrition education and training.

Now, if we can get insurance companies to reimburse doctors for dietary counseling as much as they do for prescribing meds and procedures, then we'd really be getting somewhere!
